ESSENTIAL JOB FUNCTIONS:

2.Psychiatric assessment of cancer patients and their families in the inpatient and outpatient settings including: assess, treat and monitor psychosocial and psychiatric problems and disorders.

SUMMARY

This position is responsible for providing nutrition care to patients in both inpatient and outpatient settings. Working with the Associate Director, Clinical Nutrition, Clinical Nutrition Specialists, Clinical Dietitians, and Dietetic Specialists to provide care during all stages of treatment, and for working within the team to attain departmental goals.

Specific responsibilities include:

This position is responsible for providing direct nutrition care to patients with complex medical conditions. This includes communicating recommendations for nutrition intervention in the medical record and on multi-disciplinary rounds; providing nutrition education to patients and health care professionals. Specific responsibilities include evaluation of medical status, interpretation of laboratory values, assessment of a patient's nutritional status, and recommending, implementing, monitoring and management of nutritional intervention. Provides clinical leadership to Dietetic Specialists and Clinical Dietitians and participates in development/training of dietetic interns. Provides education within the hospital and in the community and participates in department quality and performance improvement initiatives/projects.

KEY FUNCTIONS

1. Patient Care Function

(a) Provides quality nutrition care in relation to the patient's diagnosis, treatment, age group, nutrition status, and other needs in both an acute care environment, and/or within the clinics as appropriate.

(b) Provides direction to the Dietetic Specialists in the form of ongoing training on aspects of nutrition care relevant to clinical management of the Dietitian's medical services. Consults with the Dietetic Specialists as needed to provide continuity of care per service assignments.

(c) Provides a comprehensive assessment of nutritional status by synthesizing and analyzing information obtained through personal interviews, physical assessment, review of the medical record, consulting with other members of the care team and via reports (i.e. laboratory and other diagnostics) retrieved from the computer.

(d) Determines patient's nutrient needs and develops a comprehensive plan of care by using department-established guidelines.

(e) Collaborates with the interdisciplinary team individually and in group meetings in recommending, implementing, and monitoring of nutrition interventions.

(f) Documents care in the medical record using appropriate forms and/or formats within established guidelines.

(g) Provides ongoing or follow-up care according to established time frames and reassessment criteria.

(h) Identifies and provides for patient and family education needs, assesses barriers to learning, provides appropriate instructions utilizing appropriate modalities, assesses patient understanding and documents interventions on the Interdisciplinary Plan of Care and Teaching Record.
